Health Slip (required for ALL animals – vaccinated or not – this needs filled out )
Vaccination Record – LEBANON
Oregon Transportation Certificate (required for all cattle – helpful on all others )
Organic Verification Form (ONLY third party verified Organic cattle need this form)
